trak group is working with a leading behavioral health organization to find a dedicated Home-Based Therapist to provide intensive, in-home therapeutic support to children and families. What You’ll Do
- Provide intensive home-based therapy to children and families in their homes, schools, or community settings.
- Conduct assessments to identify client and family strengths, needs, and challenges.
- Develop and implement evidence-based treatment plans, including trauma-informed and cognitive-behavioral interventions.
- Coordinate with care teams including case managers, psychiatrists, and external community resources.
- Participate in an on-call rotation (once every 5-8 weeks) to provide crisis support.
- Complete required documentation in compliance with HIPAA and agency standards.
What You Bring
- Master’s degree in Social Work, Counseling, or a related field.
- Active LSW, LPC, or higher licensure in good standing.
- Preferred: Experience providing home-based mental health services.
- Passion for working with children and families.
- Ability to work flexible hours and travel to client locations as needed.
