trak group is working with a mission-driven behavioral health organization to find an In-Home Therapist to support children and families in Portage County. This role is ideal for someone passionate about making a difference in underserved communities and who thrives in a flexible, relationship-centered environment.

- Clinical support – rotating on-call schedule and access to a backup clinical manager.
What You’ll Do
- Provide in-home therapy to children and families in the community.
- Manage a small caseload, focusing on individualized treatment plans and relationship building.
- Offer crisis support as needed, with most de-escalations handled by phone.
- Collaborate with families, schools, courts, and other agencies.
- Complete timely documentation including assessments, progress notes, and reports.
- Participate in staff meetings, trainings, and other client-centered activities.
- Implement trauma-informed care models with fidelity.
What You Bring
- Bachelor’s or Master’s degree in Social Work, Counseling, or a related field.
- LSW or LPC required (independent licensure preferred).
- Valid Ohio driver’s license and insurable driving record.
- Comfortable working afternoons/evenings (70% of the time).
- Ability to be on-call for your own clients during the week; participate in rotating weekend coverage.
- Proficiency in Word and Electronic Health Record (EHR) systems.
