Responsibilities include:
- Providing comprehensive care coordination for patients with active IV antibiotic orders from Infectious Disease physicians.
- Assessing patient's plan of care and implementing, monitoring, and documenting utilization of resources.
- Evaluating medical necessity and appropriateness of care to optimize patient outcomes.
- Serving as a liaison for referring physicians and connections to community resources.
- Educating patients and families on diagnosis, course of care, medication reconciliation, and nutrition.
- Collaborating with multidisciplinary teams for timely screening, intervention, and increased supportive care.
- Assisting patients, families, and caregivers to overcome barriers to quality, medical, and psychosocial patient care.
What you need to succeed:
- Nursing license in good standing.
- Infusion experience OR recent acute bedside experience (no LTC or behavioral health).
- Strong computer skills (Epic experience preferred).
- Ability to administer flu shots.
