Responsibilities include:

Conduct primary care rooming processes and relevant medical procedures for both adult and pediatric patients.
Follow protocols and policies for clinical procedures and equipment usage.
Document patient care accurately and comprehensively.
Address patient messages promptly and escalate issues as needed.
Participate in patient-centered medical home team activities, including pre-visit planning and daily huddles.
Support outreach campaigns to close gaps in care and promote wellness.
Utilize population health knowledge to support value-based contracts and quality initiatives.
Engage in post-discharge outreach for identified at-risk patient groups.
Provide patient education and coaching for wellness and chronic disease management.
Facilitate follow-up care and referrals as needed.

Demonstrate age-related competencies and experience with various age groups.
Understand recommended screenings and chronic disease management processes.
Familiarity with patient-centered medical home principles.

What you need to succeed:

Comfortable with blood draws and injections.
Strong computer skills, with Epic experience preferred.
Ability to handle phone calls professionally.
Up-to-date flu shot.
