Job Description
trak group is collaborating with an impactful organization to find a Rapid Re-Housing Service Coordinator in Covington, KY. This role provides a meaningful opportunity to help individuals experiencing homelessness secure housing and work toward long-term stability.
Responsibilities
Service Coordination
- Manage a caseload of up to 25 participants, developing individualized action plans to secure permanent housing and stable income.
- Conduct assessments to identify participants’ needs and address housing barriers.
- Collaborate with participants and landlords to resolve tenancy issues while maintaining confidentiality.
- Conduct home visits, review budgets, and provide coaching on self-advocacy and tenancy skills.
- Facilitate access to community resources, government services, and employment opportunities.
- Ensure compliance with documentation standards, including timely data entry into agency systems.
- Advocate for participants and provide transportation as needed.
- Assist participants in finding housing, scheduling property showings, and building relationships with landlords.
- Recruit landlords to participate in housing programs.
- Maintain strong relationships with community service providers and attend relevant meetings.
Qualifications
- Education: Diploma or GED with experience in social services. Associate’s or Bachelor’s degree in Social Work preferred for less experienced candidates.
- Experience: 1–3 years in social services, with a focus on housing or case management preferred.
- Advanced communication and conflict management abilities.
- Strong organizational and documentation skills.
- Knowledge of community resources and mental health/substance abuse services.
- Ability to work independently and as part of a team.
- Reliable transportation.
- Proactive and self-motivated mindset.
