- Assist with resident admissions and develop post-discharge plans.
- Coordinate a quality assurance program for social services.
- Act as a liaison with community agencies and handle abuse and grievance complaints.
- Maintain regular contact with residents and their families, scheduling individual conferences.
- Participate in pre-admission screenings, care plan conferences, and counsel residents, family members, and staff.
- Qualifications: Bachelor’s degree in social work (BSW) or related field, or a Master’s in social work.
- Licensure: LSW or LCSW in good standing.
- Experience: Minimum one year in a long-term care setting.
- Strong interpersonal and clinical skills.
- Ability to handle sensitive issues with professionalism and care.
- Experience establishing therapeutic relationships with residents in a similar environment preferred.
