Responsibilities
- Facilitate admissions and create post-discharge plans tailored to residents' needs.
- Oversee abuse and grievance complaints, collaborating with local social service agencies to ensure resolution.
- Coordinate and participate in care plan conferences, maintaining regular communication with residents and families.
- Develop and maintain a quality assurance program for social services.
- Assist with pre-admission screenings and maintain knowledge of community resources for comprehensive resident care.
- Provide counseling to residents, families, and staff, fostering a supportive and therapeutic environment.
Qualifications
Must-Have:
- Bachelor’s degree in Social Work (BSW) or related field.
- LSW or LCSW licensure in good standing.
- Experience in a long-term care setting or nursing facility.
- Familiarity with long-term care regulations and processes.
- Skilled in building therapeutic relationships with residents and families.
