Responsibilities include:
- Provide nursing care services to assigned practice(s)/specialty(s).
- Conduct bio-psychosocial telephonic assessments using approved clinical protocols.
- Triage patient needs and determine the best course of action or patient disposition.
- Serve as a patient/caregiver advocate and communication link between the patient/caregiver, physician, pharmacy, and hospital.
- Utilize best practices in documentation to ensure efficient, concise, and timely communication.
- Aim for first call resolution, addressing patient needs promptly and effectively.
- Understand and support the mission, values, and management of our organization.
What you need to succeed:
- High School Diploma or Equivalent.
- Registered Nurse license in good standing.
- Comfort with injections and blood draws.
- Strong computer skills (Epic experience preferred).
- Acute care, case management, or physician office.
- Flu shot required.
